Disfunció endotelial pulmonar i complicacions trombòtiques en pacients amb COVID-19.

CAT: La infecció causada pel virus SARS-CoV-2 (COVID-19) ha estat reconeguda per l'Organització Mundial de la Salut com una pandèmia global que implica una important amenaça per la salut pública.

SARS-CoV-2 infecta les cèl·lules humanes a través de la fusió amb la seva membrana i inicia el procés de replicació per augmentar així la seva càrrega viral.

Inicialment, la COVID-19 ha estat considerada una infecció respiratòria que pot arribar a causar una pneumònia. Tanmateix, en casos severs pot arribar a desencadenar en una malaltia multiorgànica. La transició des de la infecció respiratòria fins a l'afectació de diferents òrgans, és a conseqüència de dues de les principals complicacions de la COVID-19. D'una banda, una reacció inflamatòria descontrolada del sistema immunitari i per un altre, la formació d'un gran número de trombus que poden causar infart de miocardi, ictus i embòlia pulmonar.

L'endoteli pulmonar participa activament en aquests dos processos, esdevenint l'última barrera abans de l'entrada del virus. La publicació de la Dra. Olga Tura al American Journal of Respiratory Cell and Molecular Biology examina la funció de l'endoteli pulmonar en resposta a la COVID-19, l'existència potencial de biomarcadors i el desenvolupament de noves teràpies per restaurar l'equilibri vascular i per protegir /tractar els pacients. A més revisem les complicacions i les seqüeles observades en els pacients amb la COVID-19.

ENG: SARS-CoV-2 has been recently recognized as the cause of a global pandemic by the World Health Organization implying a major threat to the world public health. SARS-CoV-2 infects host human cells by binding through the viral spike proteins to the angiotensin-converting enzyme 2  receptor, fuses with the cell membrane, enters and starts its replication process in order to multiply its viral load. COVID-19 was initially considered a respiratory infection that could cause pneumonia. However, in severe cases, it extends beyond the respiratory system and becomes a multi-organ disease. This transition from localized respiratory infection to multiorgan disease is due to two main complications of COVID-19. On the one hand, the so-called cytokine storm: an uncontrolled inflammatory reaction of the immune system in which defensive molecules become aggressive for the body itself. On the other hand, the formation of a large number of thrombi that can cause myocardial infarction, stroke and pulmonary embolism (PE).The pulmonary endothelium actively participates in these two processes. The review of Dr Tura published in the American Journal of Respiratory Cell and Molecular Biology examines the role of the pulmonary endothelium in response to COVID-19, the existence of potential biomarkers and the development of novel therapies  to restore vascular homeostasis and to protect/treat these patients. Additionally, we review the thrombotic complications recently observed in COVID-19 patients and its potential  threatening sequelae.
